You don't need any shots at all. None are required by the Thai govt. Your visa will be issured at the airport when you arrive. Of course you need your passport and make sure that there is at least 6 months left on it You need a paid ticket leaving the country. A pocket full of money and a song in your heart. Thailand is a blast. Don't worry so much. you will have a great time.
PS; The lady above had a good thought. Make sure your credit card people know that you are leaving the country and where you are going. Its really a pain when they cancel your card because it is suddenly getting charges from Bangkok. A pain in the rear to clear it up by international phone calls.
Keep copies of your birth cert, passport, tickets and drivers license in several places in your bags and purse. If the worst happens and you lose the lot. at least you will have the copies and a place to start. These tips are important! don't forget.

If you're planning to stay in Thailand for only three weeks, then a tourist visa is enough (a tourist visa will enable you to stay in Thailand for a maximum of 30-days). No vaccine or shots needed when you're traveling as a tourist in Thailand (unless you are coming from a country where there is a known outbreak of a contagious disease, like ebola virus or bird-flu). You need to apply for a tourist visa at the nearest Thai consulate or embassy within your country. You also need a plane ticket, your passport, and enough cash or traveler's checques. You need to make a hotel reservation beforehand (through the internet), to avoid the hassle of looking for a place to stay once you arrive in Thailand.

That's all about what you need. Good luck and have a nice holiday....

Lots of shots!........ Just read your detail..... Make sure everyone close to you has your travel plans... Let your credit card company's konw you will be traveling out of the country and carry every phone number to those company's in case something like your wallet or purse gets lost or stolen... Know what kind of food you can tolerate and search local thailand sites where you can get that food otherwise you'll be miserable...
