What it means is that you are watching a Hollywood movie (most likely a Western) and someone just got shot in the gut.
Likely he whispered a few profound utterances to his friend holding his head up off the ground prior to his eyes rolling back into his head.

Often it is because their blood is having trouble clotting normally. Coughing, and even normal breathing, can break tiny blood vessels (capillaries) in the lungs. Usually there is no blood loss though. The blood clots quickly and the capillary heals.
If a person is missing a clotting factor (one of several components in the blood that make it clot when the body is wounded), they may cough up blood, or have wounds that just won't form a scab.
It could also be a respiratory problem. Some sort of open wound in the lung(s) or trachea that is losing blood. The blood collects in the lungs, which signals the body to cough, to clear it out.

If they are dying then the fine details don't really make much difference. There is blood which could come from lung problems, stomach problems, or any trauma in their chest. The medical examiner will be taking a look anyway so it will not remain a mystery.

I am a CNA, and coughing up blood can mean a serious indication they were severe alcoholics. And a alcoholic that is dying and coughing up blood does it for months, they hang on and on, pure agony to see it.

When my totally athiest aunt died at 34, in her final hour she believed that john the baptist had come to take her by the hand and told her not to worry that peole were waiting for her. She was in alot of pain previously and for those two minutes when she spoke she really believed, and she didnt cry in pain anymore.She died within an hour of that and for the first time she didnt need a top up of morphine, it was really peaceful and she was ready .

If its while they are dying than its not bad per say their dying. Its most likely internal bleeding, an upper GI bleed, TB, esophageal varacies.

That they are "in the throes of death",meaning all their functions cease...pulmonary,respiratory,etc..but then again the bleeding could signify a wide variety of symptoms at death.If they have lung disease,or have internal injuries or etc...etc..

What would it matter @ that stage? Medically it could be a hemorrage taken place.
