You may have to go to your local Social Services offices and apply in person. Many states (including NY) don't put the application online.

find the url to the louisana government offices. Google louisiana goverenment or louisina medicade. It shoould be louisiana.gov. or google medical assistance. Then look for the forms for medicade

I realize it it too much effort to got to the office in order to let me have the privilege of supporting you. If you want to suck off of the taxpayers please put a little effort into it.
