It isn't gross; it's just sad. Someone may have abused this kitten or, well, some kind of trauma happened, because it sounds like a fractured rib. If her anus is inflamed, that too could be a sign of abuse. I'll leave that one to your imagination.
You don't have to be cat smart. You just have to be the caring, compassionat person you obviously are. There are two things you can do. The first is you can get the kitten to a vet for an assessment. She surely does need veterinary intervention, and if you can't afford to pay him, see if you can't help out in his office to work off the bill. It really is satisfying work, because animals are always grateful and loving, and they are interesting. And animals skills are worth GOOD MONEY. So you wouldn't be losing in any way.
Once the vet has assessed her, if he determines that indeed she was abused, you need to report this to the police or if you have an ASPCA in your community, report it to them. Have you ever watched Animal Cops on Animal Planet? If the vet determines that she is the victim of abuse, that is who you need to report this to. Because a rib fractured through abuse is a bad enough thing, but abuse of her little hinder is a very bad thing, and if it is abuse and was done intentionally, the person who did it is a potentially very dangerous person. The cops or animal cops in your grandmother's neighborhood should be aware of this and be on the lookout for other instances of the same behavior.
I am only guessing here, but a vet could tell you for sure or for pretty sure. And it is possible that, if she is an abuse victim, the vet will give free care, or reduced-cost care. But get her to a vet right away. She needs to be assessed.
And tell your dum-dum uncle that that little kitten could be a true angel. Her little body (which he finds so inconsequential and repugnant) just might help the authorities identify and nail the very worst kind of sicko, before he (or she) moves on to bigger prey.
Get her to the vet, sweetie. It is important that she be assessed immediately. Okay?

OK Hun, calm down. If this kitten is over 4 months old, and she seems to feel fine, she plays, eats, drinks, etc., and meows A WHOLE LOT, what you have here is a little girl in heat. If you are planning on keeping her, get her to your vet to get spayed. Yes, they do swell up, and in some very young, inexperienced females (in their first heat), they will drip blood a little bit.
Now, if this is just a very small kitten (8-12 weeks old), she still needs to be seen by a vet. She could be infested with worms to the point that her butt is getting real sore, possibly infected. Before trying to take this responsibility on yourself, get permission from your grandma and/or parents for you to keep her. Otherwise, take her to your local animal shelter and tell them that some idiot dropped her off outside your grandmas house. They have vets at the shelter, and this kitten is sure to get a good home, as calico's are very much in demand.
Now--PLEASE pay CLOSE ATTENTION to what Mary C has to say---she is absolutely right! Actually, my first guess was abuse, but then I thought, No--not to a kitten--but, sadly there are some very sick, demented people out there! Where I live, we know of a guy who will (don't quite know how to word this), but let's just say--he will abuse a CHICKEN to DEATH! These people need to be locked up and the key thrown away before this escalates into abusing children. So, Please DO pay close attention to what Mary C said, I think she's right. Best of luck to you with this little one, honey---and you do NOT need to be a cat expert to have a very loving, devoted kitty yourself. Cats are VERY easy to care for--much moreso than a dog. And, if you do get permission to keep this baby, PLEASE keep her as an INSIDE ONLY cat. It is quite common for people to steal calico's.
Best of luck to you with this little one, and I surely do pray that abuse is NOT the case, as this could have completely torn her up inside, and I cannot even imagine this, but I know that it happens. Also, DO call the police about this, call the ASPCA, Humane Society, and your local animal shelter--they have vets at the shelter who just may take pity on this poor baby and treat her for free.

Take him to the vet... Fast. Don't let your uncle do anything. If the cat does have a disease then you don't want to leave it untreated any longer. Then just do what the vet recommends. Good Luck!

If it's sticking out it's a prolapsed rectum which the vet can fix but you can't, it's delicate and needs to be done by a professional.
It's totally treatable, I don't know why your uncle is being a pain about this.

She needs urgent medical attention. If you can't take her to a vet (which considering your uncle wants to euthanize it himself... I doubt you can)... Please contact your local Humane Society which can be found at www.petfinder.com and they will come and collect her free of charge. They can they either give her the medical attention she needs or... put her out of her misery professionally and humanely.
