Technically, only a pathologist can make the diagnosis after a surgeon does a biopsy of the suspect sight. That means the surgeon cuts out a piece of the thing (with a few exceptions) and the surgical pathologist will make a slide, look at it under a microscope, and give a report with the diagnosis including WHO grading.
How does the surgeon know to biopsy? They typically are referred to by a physician who has gotten some questionable results from blood tests, histories, and/or imaging.
With regards to how you get them, sometimes you know and sometimes you have no idea. It's a tough thing to tell.

They might feel a tumor or you could have another symptom that send you to the doctor. The doctor thinks it is suspicious. He might do an x-ray, then a CT scan. He might still be suspicious and they think they can do a biopsy. The biopsy is the one that gives the diagnosis. From there, they might do a PET scan to see if there is more and where. Then you discuss treatment. Cancers can come from having bad habits (like smoking), it can come from toxic substances in the environment. There are many ways to get cancer.

First, the syptoms checked through a set of exeminations. The evidence of sitology etc. Then the biopsy occured. It depends on the result and the type of the cancer. It then, this is not enough, further exemination is checked through lymph cell biopsy or cell membrane layer per layer. Then the staging of cancer is acquired. Blood test also checked to know the evidence of the tumor marker. usually done first before any biopsy is decided to be done.
It depends on the cancer and the evidence of imunohistology of virus or bacteria, the medical professional will inform further. But usually, either genetical or degenerative, life style ( food, activities,habits or addiction and emotional management).

being diagnosed is usually blood test and mri's and things. the doctor will take your symptoms and match to what he thinks is right. that is usually the LAST thing they look at unless ur symptoms are too obvious and specific. cancer is a weird subject cause mannnnny things can cause it. environment, food you eat or don't eat, and genetics are top factors.
