who was difficult to understand?
I'm from Texas, and I once met someone from North Carolina and I was amazed at that wonderful accent but could not understand a lot of her conversation. BTW, my favorite person in the world is from North Carolina so I'm not bashing the accent. Also, people immediately recognize I'm from Texas when I go to other states so I know I have the Texas twang.

Yeah, actually I remember once my family was in Texas, and they didn't understand the waitress and the waitress didn't understand us at first, so we had to "start over." It was a good laugh.

In my state though, there is a section known as Amish Country and Pennsylvania German English sounds very funny. If you don't know any German, you might not get any of the idioms and little grammar in their accents.

Also New York has some thick accents in some parts as well.

I am a professor, and I have had students from Mexico, Brasil, Argentina, Haiti, Venezuela, Colombia, Peru, Uruguay, Jamaica, and other countries from America. Sometimes we use the same words with different meanings. That is confusing indeed

Yes, in several places. The strongest American accent I experienced was in Hawaii. Many Hawaiians are actually bidialectical; They speak standard English with mainlanders and the local dialect with friends and family. Your observation is valid in many parts of our huge country.
