You answered your own question. It's carpal tunnel from all the piano playing, just give it a rest or you'll eventually need surgery.

I would first check with a doctor about this just to make sure that there's nothing else going on. You may have also done damage to not just the carpul tunnel nerves, but to the other tendons, bones, and ligaments. Secondly, by all means, rest your wrist, or at least put it in some type of brace to give it some support.

I'm not a pianist, but I am a flutist/piccoloist, and was really down the road on developing carpul tunnel/tendonitis. Like you, I was practising 4-5 hours a day at times. What has also REALLY helped me was studying the Alexander Technique (which I'm now working towards my certification in). The Alexander Technique is method of re-educating yourself through everyday activities to lessen and prevent injury. Many times, pain is caused by how we use ourselves as a whole. How we stand, sit, brush our teeth, wash dishes, type at a keyboard, exercise, and even play a musical instrument greatly influences our tension patterns or habits. Simply put, if you use a lot of tension, your body is going to revolt. However, you don't want to be all floppy as that's just as bad for the body. Overtime, these habits cause a lot of strain on ourselves, causing a whole range of physical problems.

Although it's not a medical "cure-all," it can be of great assistance in relieving your pain. Feel free to check out the sites below for additional information.

Principle Causes of Wrist Pain

There are two principle reasons for wrist pain. A sudden event or an acute injury can break a bone or tear a ligament. Less apparent, but more slowly developing insidious causes of wrist pain are repeated patterns of activity, which over time may cause undue strain on the wrist. What can result is commonly referred to as Repetitive Strain Injuries, which include (1) Carpal Tunnel Syndrome, and (2) DeQuervain’s Disease.

Other potential causes of wrist pain include diabetes, thyroid disease, arthritis, and pregnancy (which can cause severe swelling and other wrist problems).

Traumatic Arthritis

The healing process which results from the traumatic stress of a sudden injury often causes arthritis. An accident occurs, and the bone or the connecting ligaments are broken or torn. After the broken bone fragments are realigned, even if the realignment appears to be perfect, there will still be some distortion. Over time, the broken bones will settle, increasing the distortion, and the surfaces of the bones will tend to rub and wear out. The cartilage lining on the end of the bones is especially susceptible to this “wearing out” process, and as the tissues and linings around the bone become irritated and swell, scarring will eventually occur. All of these relatively small changes will cause a greater degree of stress over time on the bones and the cartilage, and inevitably arthritis will result.
