At death our physical body returns to the earth from which it was made and our spirit body, containing our personality and memories of earth life will go to the spirit world to continue to learn and progress. At some point our spirit body will be reunited with a perfected physical body in what is known as the resurrection.
This isn't just speculation on my part. Our Heavenly Father, the father of our spirits, once again speaks through living prophets, helping us to understanding the scriptures and revealing additional information about the purpose of earth life. There are some short videos that go into more detail at:

When I die, others will still be alive getting on with their lives. Perhaps I'd like to think some may mourn my demise, but that will depend on what I did in life. When I die, I shall be no more, so I won't be conscious and therefore I won't be in a position to know what happens following my death. So there is no point worrying about it.

Like it or not, we human creatures are animals, we are primates. When one of us dies, decay begins almost at once. If a mortician has done whatever it is they do on your body, your remains will linger but soon enough the process will resume. Within a disastrously short time, depending on the climate, the kind of coffin, the depth of the grave, the environment, your remains will have wasted away with help from insects and sometimes animals. Left will be some bones, the skull, and some rags. There's nothing new about it, nothing romantic either, and most decidedly nothing religious. No one, by which I mean, No one, has ever risen from the dead. And no one ever shall. We are simply animals, we are primates, we are born of a male and female, and in a short while we die. Period. Period. To think otherwise suggests one is not 'thinking' at all.

When you die, your body gets laid in the grave. Your spirit separates from your physical body and goes back home to that God that gave it life. You will go to a wonderful place, a heavenly place that is far more wonderful and fantastic a place as this earth. There, you will be greeted by friends, associates and family members who have gone on before you. There, you will await the resurrection of the body, made possible by Jesus Christ. When you become resurrected, your physical body and spirit body will be reunited again, never to be divided any more. As a resurrected person you will be judged of Christ, and assigned to that heavenly place that you will ultimately live in the eternities. It will be a wonderful, heavenly, fantastic place that will be glorious.

Having never died, I can't say with any certainty, but I believe that upon death, all of the electro-chemical activity in the brain ceases, causing all that there was of the personality to be anihilated.
