RE:
How many square feet of concrete 4 inches thick could be poured with 1 cubic yard of concrete?

Well, first get common units...
1 cubic yard = 27 cubic feet
4 inches = 1/3 foot
Since Volume = Base * height, where Base is in square units, you can substitute in the values to get:
27 = B * 1/3
By multiplying both sides by 3, you get:
B = 81 square feet
Hope this helps.

All the online calculators confirm that you need 81 square feet of pour space, but you have to remember to add in the space taken up by rebar or any re-inforcement and deduct for shrinkage. If you don't you will end up with 3-1/2 inches after shrinkage depending on the amount of gravel, sand and mortar in the mix. So, use an online shrinkage calculator and work backwards from the amount of finished volume (space less reinforcement metal) like the one at http://www.practicaldiy.com/general_building/concrete_calculators/concrete_calculator_2_i.php
A finished space of 1 cubic yard you will see requires 1.56 cubic yards of sand and gravel using the default settings.

This is a very simple unit conversion problem.

1yd^3 x (3ft/yd)^3 = 27ft^3

Convert 4in to feet:
4in x (1ft/12in) = 1/3 ft

Now divide:
27ft^3 / 1/3ft = 81ft^2

36x36x36=46,566 in sq
4/46,566 = 11641.5 (take the sq root of that )= sq in. of concrete.
4" x 108" x 108" = 46,566 in sq
