Get her a locket. After you take a picture with ALL 3 of you in it. (Or whoever you want to be in the picture) Go to the store or edit it yourself and make it small to put in the locket. They have a variety of lockets at the store. Including some that say "MOM" or "WE LOVE YOU" or something like that.
You can also buy a ring or necklace with the birthstone of your child. (Based on the month they were born)

A spa gift certificate.
Clean the whole house (really really good) for her.
A designer purse. Coach or LV (She would LOVE this)
A pair of diamond earrings or a pearl necklace.
Something you can give her that's free: A break during the middle of night feedings. Even if she's breast feeding she can pump enough for the night. She will forever thank you for those nights that you let her get a couple hours of extra sleep.

I bought my wife two things. First I bought her a Coach diaper bag and she loves it. Second, I bought her a tag bracelet from Tiffany's. Once we had the name, weight and date of birth we brought it back to have it engraved with our daughters iniitals, birthweight and date.
If you are going to get her jewelry make sure it has something to do with a baby.

Our first born arrived, as so many do, at 2am in the morning I arranged to have a catered dinner, complete with wine and music in her room at the hospital the next night. I filled the room with fresh roses, Held her hand and spent a nice evening just the 3 of us.On the birthday of each of our 3 children, I have a dozen roses delivered to her at work with a photo of us holding the child who's birthday it is.

A piece of jewelry with all 3 of your birthstones...the baby's in the middle. Someone else suggested something similar...it's nice when you get something that can be added to as you have more children. You're so sweet!
