They're referring to their bone structure, not their actual weight. This can be gauged, to some extent, by simply placing their hand around their wrist.

women are very selfconcious, so it makes them feel better to say that, and its rude of you to tell them they are fat. sometimes they honestly can't help it ( geneticly ) so it doesn't help for you to say that to them, except people for who they are without prejudice.
