Most of the smaller ones that do that number of pages are actually drills rather than punches. When you put in the paper you can actuate a drill to go through the paper in the proper place to match up with a binder.

If you were using an actual punch for production it would be equivalent to a sheet-metal punch. A motor driven flywheel would turn a big cam and the cam would have one position in which it presses on a sharp punch that goes through paper.

Depends on the model. There are electric punches that can punch holes in steel! An electric punch has a mechanical system of levers and cutters driven by an electric motor. I've searched the 'net but can only find ads to buy one and no info on how they work. I know from experience that they have a cutting table with an index and a cutting area where the holes are punched.
