Unfortunately, yes. All smooth muscles in the body relax at the time of death causing involuntary defecation and urnination too. I've seen this happen when a ladies dog got hit by a car and he died on the road as I came on the scene. It's no different with humans.

My mother died recently aged 97 with advanced dementia. I was there and although she d been unconscious or semi conscious, her face contorted terribly just as she died, as if she was in a lot of pain for a moment. It has worried me very much but now I am wondering whether it was simply the passing of a hard stool, since she had had no fluids for a few days. Could that be right?

Yes. My wife's sister is a nurse that worked in a nursing home. She said whenever a patient died the first thing they did was pack the rear ends with gauze or they would end up with a mess. Sad but true. No pun intended.

It is true when you die all your muscles relax. If there is material in your colon then it does empty.

Yes, and the bladder too. I worked in ER on a code team and it does happen. It also can happen when you have a seizure, pass out or faint.

Your muscles relax, and it all comes out, one way or another...it's your muscles that hold it all in. So yes, things get messy.
