I can relate with your man, I don't usually celebrate my b-day either, and I always tell people not to get me anything... but in all actuallity I really want them to acknowledge the unsubtle requests I've made throughout the year. For example, I say I don't want anything, but I'm also always talking about how much I need a new shaver.
Basically, he doesn't want anything for fun he wants things that he needs but doesn't want to spend money on them such as a new pair of workboots, or a new radiator. The reason someone says they don't want something is because they know that you'll never guess what they want so in order to not be dissappointed, they rather not get a thing.
I bet you, if you rummage his living quarters, you'll find something that is getting worn out or you'll find what he's been missing. By you paying attention to his "NEEDS", that's going to show him that you care and ultimately THAT should be your gift to him!!

A simple dinner at his favorite restaurant. Take him at his word, if he says he doesn't want anything, don't get him anything. If he works all the time, he probably would just value time with you over anything you could buy him.
And tell him happy birthday for me. You're a good person for being concerned about it. But the old adage is true...the closest way to a man's heart is through his stomach. Good dinner, good company, what else would be needed?

Favorite sports team? even if you get tickets for a game thats far away, he could enjoy that. How about something you make? I made a fleece blanket that my bf sleeps with everynight. It's good for someone who has "everything" because he couldn't possibly have something I haven't made yet! How about a trip, or a package for a beer manicure? How about throwing him a b-day bash with all his friends? There is also this great site called FindGift.com where you can search for gifts from everywhere by personalizing your search with the person's age, occasion, relation to you etc. I've found great age appropriate, sex appropriate, occasion appropriate ideas on that site.

Get him a gift certificate for Victoria's Secret and go down there with him so you can model his choices for him. Then take it all home and have fun!

Is he a sports fanatic? Get him a sports tee. Like music? Get him a cd that he likes, and maybe hasn't gotten yet. How about a necklace.

Invite him over to your house for a get-together; (his favorite movies & pizza) of you both love to "party" involve some alcohol or beer, or go to a favorite club. (since he says he doesn't want any "gifts" or "presents".) -but whatever you decide.
