Very very carefully. The trick is in having the bottle-top grabber sewn into the bra so that it cannot be seen if someone is looking down the blouse during the trick and ignoring the obvious diversions.

Just push the bottle (twist off) against your boob and twist the bottle. Can't be done with a regular bottle. No biggie...see this video:

I would assume that you would wear a low cut shirt and attach a bottle opener on a tightly fitted bra just place it at an easy position to hook and pull.
