This will give you no punya. Rather you will earn sin. You have no right over your own body even. Only God is the Proprietor of your body. So, how can you donate a piece of property which is not yours?

You can do that by filling up a form. I have done this at the Hinduja Hospital. They will issue you a card which you should keep it such a place that when the time comes for your demise, you family member have to call the Hospital and get the needful done. God Bless you dear.
