Three violations in a year?? Expect a notice that the license has been suspended. You can appeal, but you're probably going to have to prove that you've fired the servers, and that you do comply with the rules. The liquor board does not take kindly to any infractions, and I'm surprised the police haven't shown up. Guard that liquor license with your life! You're paying for it, in more ways than one!

Call your lawyer or the Licensing board and ask... they may revoke you. I know in Texas serving minors is a big NO NO.

Ask every customer who looks young to show them their drivers license and remind your waiters an waitresses to be more care next time.
