All legal documents are dated Oct 2006. Someone said that I have to prepare notorised affidavit/ advt in newspapers again, since the earlier affidavit is valid for 6 months only for making change in passport. Is it so? Can anybody confirm?

Approach Commissioner of Oaths. Get your name notarised, then proceed to passport office and then apply for a change in name. Currently names are not changed or attested in your old passport. They will be issuing a new one and if you are in urgency, ask for TATKAL scheme whereby you can get your passport with new name in a week. Even your educational certificate get valid with new name only with notary certificate. As you are sixteen years, you r declaration should be accompanied by your parents or your legal guardian. The certificate issued by commissioner of oaths does not loose value by time frame- however there may be some government bureacurates who can make you running with their own imaginary reasons. If you want to oblige them and go with them hand by hand go for a new certificate (it may bill you around hundred rupees only) Decide yourself

Contact passport office and ask them for guidance in writing.

They will mail you a standard print out, or mail you a special instructions.
