I have been told that genuine Italians don't use the spoon, they twirl on the edge of the plate. I am not Italian, I used to use the spoon but now, I just take a small quantity of the pasta and sort of twirl it an inch or so above the plate.
Spoon, edge of plate, or air, whatever works for you best. If you don't splash sauce I guess you are OK!

It's rather polite, you can eat it any way. But for manners sake, if your out in public, eat with a spoon. Most people use both, because they use the fork for actually eating (putting it to their mouth) and they use the spoon for twirling the noodles around the fork. (they hold the spoon so that the hollowed inside meets the fork, and then they just start twirling the fork so that the noodles spin around the fork and into a neat ball of noodles around the fork.) It's also easier to eat that way.

The fork and spoon are used together to twirl the spaghetti onto the fork. It makes it a whole lot easier.

I think most people use just a fork. It's hard to master the spoon thing and can end up making a bigger mess.

I come from an Italian family...both sets of grandparents came from Italy and we've never EVER used a spoon. We do, usually cut spaghetti so it will be a small bite on your fork instead of a huge mouthful.

You can and you may. YOu can even just use the fork to twirl up the spaghetti.....no spoon needed. You can also twirl the spaghetti using the fork with the spoon under it.

Whoa, don't you dare touch a spoon to your spaghetti in Italy. You'll get arrested! (just kidding). Nooo, you have to practise swirling your spag around your fork and then lifting it to your mouth; and it really is ok if you have to slurrp in a couple of strands lol. With a little practise it isn't nearly as difficult as you imagine. Besides, it's that little slurp at the end that makes the meal lolol.

Use whatever helps you to eat it neatly. Etiquette is about pleasing other people's sense of decorum... if using a spoon is going to make a bigger mess (it does for me), skip it... your dining guests will thank you later.

You can but it is also acceptable to use your knife and cut the noodles into a size that would fit your fork. That way if there are meatballs those can be cut into bite size pieces as well.
