Maybe not a lesser quality from a chain, but I have had to have eg redone more often at a chain because the prescription was done incorrectly. I personally would rather have my eyeglasses done by my doctors office where the optician is more reliable than I have found in the chains.

I don't think so, I've gotten some very nice glasses from Walmart and other optical chains, just pick out a nice pair that look sturdy and that won't fall apart. I like flex frames and any kind that can sustain abuse. Just check the frames where ever you go before you buy any there. Also make sure the employees are professional.

No. You have less of a relationship with the chain than with the doctor's office, which means they may not be as invested in your satisfaction, but the glasses are made the same.

The lenses are ground to the R/X specification by the mfg, not by any seller or doctor. Most chains do discount them.
They are all pre-manufactured in all necessary specs but must be cut to fit frame choices.
The real profits are made on the frames. Most docs do not sell frames.

No. Most places (offices or retail chains) send their orders out to large factories that create the finished products. The chains can offer discounts because of the large volume they sell.

No they are not. A doctors office does not have as many choices and their prices can be quite a bit higher. It is the same manufactuere, just a different outlet.

I dont think so, actually, for your dollar you probably get better value from a chain since they have more financial backing
