You pay up front when you apply. The passport is mailed to you.

You can apply at some post offices, some libraries, and some other places - see http://iafdb.travel.state.gov/ to find a passport acceptance center near you, and http://travel.state.gov/passport/passport_1738.html for more info on getting a passport.

Up front. Then you get your passport and birth certificate in the mail. Remember, if you go to the post office to do it you need to have already filled out the form and have a passport ready photo taken.
