Normally they don't ask for an address so give them the best number to contact during the day maybe a cell phone. The person who agreed to be a reference will tell you which number to use.

You normally list the phone number and address they can be reached at during working hours so that the person that is contacting them for a reference is able to reach them.

Yes, you list the address and phone number where they can be reached. Typically, this would be work information, but if it is a personal reference or if they'd prefer to be contacted at home, you'd list the home information.

Work! Listing home information , especially without specific permission would be a breach of privacy.
