Is it a true hematoma? Or is it on the point of his elbow? If it is a fluid filled sac on his elbow, it is a hygroma and it make take cortisone for it to go away. These hygromas are most common in older, large breed dogs, expecially Labs, but they can occur in any breed at any age. Take him to the vet if you aren't sure about it.

It depends on what it is from and how severe it is. Generally, it can be anywhere from a few days to a few weeks. If it seems to not be going down, call the vet.
