"Estimado caballero/dama"
Estimado caballero, o señor is masculine
Estimada dama o señora is feminine.
In a formal setting, we use more dama and caballero; señor y señora is more informal.

Dear sir/madam = Estimado/a señor(a).
Querido is dear in the sense of beloved.
I wouldn't use it in this sense.

dear Sir = Querido señor
dear Madam = Querida señora

thanks in advance( we don't use this expression in Spanish, we simply say " thank you so much" = muchísimas gracias
