What would be the basic design requirements ?

If a a standard engine does not have enough low speed torque buy a gear reducer to go with it. Speed will go down and torque will go up as per the ratio you choose. Make sure the incoming and outgoing shafts match the engine shaft and the load shaft.

You will need some gears to reduce the engin speeds. That will give you all the torque you need.

if your talking about about a truck just put it in a lower gear higher engine rpm's and low speed= higher torque
