It's actually a mistransliteration from the original Arabic.

Mohammed was hangin' out with some of his homies and this dy-no-MITE chick walked by and Mohammed said:

"Wow! Is she phat or wha' ?!?

And it eventually became shortened to phatwha, then fatwha and now, in some 'hoods it's just "fatwa".

It means a really fly sista . . . like, "Hey, yo, she's fatwa . . !"

First it is not a "what the hell" term.
For unclear occasions, wise and knowledgeble people of Islam decide on if the action is ok or if it is sin.
So the decision is call fatwha (I don't know if it is written like this, I decided on its pronounciation).
