If so, how the heck do you do it?

i usually do, it's not so difficult once you get used to it.. but then again you can simply, take off the shoe and drive barefoot, no problem

Almost all females drive vehicles barefoot if they wear high-heeled shoes above 2 or 2.5" . They are pretty sensible drivers. Infact the better ones have a stand-by pair of sandals they change into for more comfortable or safer driving.

If I am a female I wouldn't bother about ultra-high heeled shoes because they slowly and surely stagnate the natural development of the bones, ligaments, sinews which bring about some pain to the feet and the calf muscles.

I would consider 20 other spots to attract the males, rather than the mere look of propped-up feet or swaying hips that can only unfairly cause eye-strain or vision impairment on the part of we, poor men...

I don't, I take my shoe off and drive barefoot. The heel gets in the way and I'm always afraid I'm going to jam the heel under the pedal and get stuck

I take off any heels/dress shoes and sometimes even flip flops. If it's not strapped to my foot, I don't want the chance of it getting stuck under the pedal.

I can but it's awkward and unpractical. If I'm forced to drive any distance in high heels then I take them off while driving or have someoneelse drive if at all possible.

I don't wear high heels, I'm tall and I like comfortable shoes, so no, I don't do that. Interesting answers though.

It can be done. Messes up a lot of shoes and causes more accidents than we will ever know.
