Only ten out of the first 28 answers were even partly right, and nobody had a comprhensive answer. Yes, it can sometimes be used as a guilt trip to beg for sex. As mentioned before, it isn't fatal or even really damaging. It can be very painful, and the pain isn't felt directly in the testicles but rather in the abdomen as a moderate to severe cramping. Pressure on the testicles, either external or internal, can cause it and it's the same pain in a different degree to being hit in the testicles. So it can be caused by a prolonged erection because seminal fluid is generated during arousal, enough to eventually cause internal pressure. It can also happen from external pressure, by pressing up against the partner for a period of time. There's no advance warning. It's a gradual thing and you're well into it by the time you notice it.

An observant and sympathetic partner can warn, "Careful--you're pressing too hard." It's often the result of external pressure. But prolonged arousal can cause internal pressure, up to the point where blood appears in the semen (hematospermia). So it's a very real condition. It has nothing to do with how long ago the last ejaculation was because the fluid is also continually reabsorbed. It builds up in the short term, during arousal. The longer the arousal, the more semen is ejaculated. Or pressure built up.

Blue balls come from expecting to get some and having the juices building pressure with no release. This is a real condition and can be painful. It is temporary and any guy will get over it in a few so do not allow yourself to be guilted into having sex cause of it.

I couldn't give it to you medically, but what happens is that if a guy gets aroused and it happens often enough without a release, then literally his testicles hurt. That's called blueballs. It might just be his way of laying a guilt trip on you so that you will finish whatever was started.

Blue balls is a term used when the "plumbing" downstairs starts in motion, but then doesn't get to complete the job... Clearly he was aroused and wanted to get it on, but since he couldn't clear the pipes, there was a backlog going on... It does kind of hurt a bit... Best way to get rid of it, besides sex, it to rub one out really quick...

Have sympathy for the guy...

Blue balls is a slang term for a temporary fluid congestion in the testicles and prostate region caused by prolonged sexual arousal in the human male. It is often accompanied by a cramp-like ache of prostatic congestion and pain/tenderness of the testes. While the term is usually applied to men, the female homologue is usually referred to by the more general term "pelvic congestion," or "pink ovaries."

it means you get so excited that your body is expecting to enjoy a release (orgasm). then when you don't, it is painful, to the point that even the slightest touch to the balls hurts. Most guys would eventually relieve themselves, or take a cold shower. This has happened to me before, when on a first date, I got so worked up (erection) over this girl (and we didn't even really do anything but kiss a few times) that when I got back home I had to finish the business myself.

When ever the term "Blue Balls" is used it means a guy has gone way to long without sex, and he is starting to get backed up because of no release.

When a guy goes a while with out having sex, and gets excited his balls start to ache. They don't actually turn blue, but it hurts and they need a release.

Basically it is an aching of the testes caused by built up unreleased semenal fluid. Tell him to jack off.

It refers to aching balls. The feeling a guy gets when he is SO horny but doesn't get to follow through with sex. The balls don't really turn blue. That's a myth.

There's no scientific medical proof for blue FREAKING balls.
