Hey there, have a little problem and was just wondering if anyone has/knows an explanation for it.

While speaking/typing, i always tend to occasionally misplace a word with a completely random word unknowingly. Is there some sort of medical explanation for this?

If you had aphasia, the errors would be more frequent than you describe. Everyone, without exception, will have neologisms or errors in word retrieval from time to time, just as everyone stutters occasionally. These problems are not pathologic, nor do the signify brain damage if seldom experienced.
All you need to do, really, is proofread your writing, or listen to your speech output so you can self-correct.

It seems you are describing an aphasia like anomic or noun aphasia. Aphasia, usually a symptom of some brain damage, can occur from things as simple as some learning disorders. You may want to see a doctor to find out why, depending on how badly it affects your communication with others. From your question you seem to be quite aware and communicative. It would be of concern if it has a sudden recent onset. It is common to have it to some extent as we age. A more frequent form would be to replace all nouns with pronouns making it difficult to communicate with others because they could not follow our subjects.

A sudden onset should be checked out on an emergency basis. A slow onset aphasia or muteness can be symptoms of dementia.

If it makes you feel any better, the other day i was talking about roasting marshmellows and how they will explode in the microwave, and i accidentally said mushroom. I have no clue where the word mushroom came from.

(To add to that, it turns out that marshmellows don't actually blow up in the microwave, just those "peeps" easter candies, and the like.)
