Constant supervision is the only way to housebreak a puppy. First get a crate to keep him in when you are not around or sleeping. Make sure to feed him in the crate as well (dogs do not like to go in the same place they eat and sleep). Make the crate a happy place that he can have as a safe place. Never use it for punishment. If the puppy is still real young it may have trouble holding it during the course of a full workday so make certain you get it out for bathroom breaks.
Take the puppy out after every meal, after every nap, and after every playtime. Then while it is awake take him out every 30 - 45 minutes. Do not let the puppy wander around the house without supervision. This will lead to accidents.

So before he goes out to do his thing, puts his paw on the bell. Then he will associate the two. ring bell - go outside - pee.

Don't use pee pads, they tell your dog it's ok to go in the house.
Always keep him in the same room as you so you can keep an eye on him. If he wanders too much get a leash and tie him near you so you can watch. As soon as he starts to go tell him no no no no and run him outside. When you goes outside praise him. AS he becomes more trustable you can let him have 2 rooms at a time and eventually you won't need to watch him at all.
If you can't watch him or are gone put him in a crate. Most dogs will not go where they sleep. This will also set your dog up for success. Since he can't go in the house because he is in a crate or being watched and corrected then he won't have a chance to get away with and think, well, if mommy's not home I can pee on the carpet! and it will take much longer to re-train the more often he goes.
Do not leave his water or food out. After he eats or drinks find out how long it will take for him to have to go. Then get him outside when he'll need to go and reward him for going potty outside.
Take him out before bed and before going in his crate, and don't give him food or water 2 hours before bed or being in a crate for a few hours.

This worked 4 my puppy i hope it will work for yours!
1.when ur puppy is done eating take it outside and while ur goin outside ask it if it has 2 go pp.
2.when ur puppy does it's business praise it.
3.if it does go in the house scold the puppy and cover the smell up w/ freebreeze or somethin so it doesnt leave it's scent
4.try to take it 2 the same spot everytime when u take it outside

My response is much in line with Debbie V...Crate or kennel training is the best approach I have used for several reasons. 1 as Debbie said it teaches them to hold it because they naturally do not like going where they sleep. 2. The crate/kennel becomes their personal cave or safe place and they grow to be very comfortable here...never punish in any way you pet while in his/her safe place. When I am focusing on the house training I open the crate and receive the pup as he/she exits with loving embrace which helps the bonding as well as distracts from what’s the immediate on their mind...relieving themselves. I carry the pup petting and loving on her all the way outside and then put her down which she normally will then go back to business at hand and go potty. Immediately I applause her and walk her back in directly to the reward zone where she receives a doggie treat for her accomplishment. I begin treating her for all instances done outside till she gets the program menu down pat. Then I cut treats back to only if they do their poop and I tell them if they do poop they will get a cookie. With my personal Shepherds I took then a step further and taught them right paw was yes and left was no. I would bring them in ask if they did poop and get them to give me the yes paw. Then I would ask them if they wanted a cookie...again they were taught to answer with the yes paw. Once we got house breaking behind us and began working on the No answer by asking if the did poop when they hadn't...by learning that when they did not and answered correctly they found they still got a reward cookie. I have two females now and both can fluently answer a simple yes or no question through this method of teaching...I even try tricking them which worked a couple times by asking if they wanted a little cookie, they said yes so instead of getting the nice big peanut butter favored cookie they were accustom to they got just a plain little ole cookie out of a jar right next to the ones they like. Didn't take long for them to think it out and answer no to my question regarding a little cookie so I would then ask if they wanted a BIG cookie and they would slap that yes paw up there...from there I have found I can now communicate with my dogs and they can express what they do and do not want. Never under estimate your pets capacity to learn...his/her only limits are within your own imagination

ARe you using a crate? Unless you are specifically watching him or playing with him, he needs to be in the crate. That teaches him to hold it.
Before breakfast
After breakfast
after nap
walk
before dinner
after dinner
Your pup should learn to hold it, because he's in the crate, but when you take him out of the crate, you have to assume he needs to go. If you catch him going in the house, tap his nose and say No, Noah!

If your puppy isn't getting it, that is a sure sign that you are not doing it correctly.
If you are not already using a crate, you better get one. If you are already using a crate, you probably need to be more consistent.

puppy pads they have a stench that housbreaks your dog to them and then you can bring it outside whenever you bring her/him outside to use the potty to sniff it, once it smells that stench outside over a number of times it will associate outside as potty and use it eachtime, it works wonders!!!
