We are staying for 10 days on a private villa and not at a hotel. Should we get a car for maybe 5 days and then use taxi's the rest?

yes....taxis and buses are good, but a hair expensive....
Antigua ( like here in St Thomas) drives on the left....which is weird enough for an American, but also has mostly right hand drive cars which is even weirder......so ask your rental car company for a left hand drive which will lower the 'culture shock' a bit....

Yes, rent a car. Gives you freedom to come and go as you please. Driving on the left is not bad. You will get use to it in no time. Just take it slow.
