Firstly is it an original speedo? If not, then its not reading true.
Second does the mileage match the age and condition of the car? If the car is in mint condition and the mileage is low, then its probably true. If the car's in a rough condition but the mileage doesn't reflect the years of usage then it may have been tampered with. I don't know if they still have it, but there used to be a security wire with a numbered tag soldered onto the odometer part. But you would have to remove the odometer from the dash to check if it was still intact.
The other option is to have a car inspection done before purchase. Some motoring clubs do these (Automobile Associations) That should give you a good idea of how much use the car has had.
If a car is sold second hand the dealer/seller must state if they believe the odometer reading to be true, and its a crime to make a false declaration there.
Otherwise you just have to go with a seller you feel you can trust.
