download Adobe Reader 8
open the PDF file
cl the Help button in Adobe
click the Index button
scroll down to the bottom and click “F”
near the bottom is an option for forms
instructions there

The only one I'm familiar with is Adobe Professional which has Live Cycle Designer that does that. I ain't free either.
