It doesn't come from SS. It's usually a private insurance you purchase to supplement, and the cost depends on several things. It depends the insurance itself, the type of coverage - if you want to be bothered with deductibles vs. an HMO-stle plan, major medical or hospitalization, prescription coverage or not... Same as it would be for regular medical insurance.
