Never add water. There are two thing you can do.
1.) There is a solvent you can put in the ink to revive it.
2.) Buy more ink.

I have put water in ink before while doing calligraphy it worked a little while until the water settles from the ink giving you transparent strokes and the ink consistency is no longer the same.

Place the bottle in hot water and let it warm up. Than ad some drops of water at a time. Use a brush to make it liquid again. Do not 'scrub' but gently brush the dried ink so it will dissolve and not flake.

Mostly it will work but some inks will not get back to being good inks.

If you mean ink in a bottle, then yes. But don't add too much at a time. If it dried up a long time ago, the pigments might have deteriorated and won't write very well.
