It's called a 1040, 1040A, or 1040EZ. You need to file a tax return showing that you are owed a refund to get a refund.

You need to file a tax return for the year, showing your income, withholding, and various other information. The main form is a 1040 - it comes in three versions, 1040EZ, 1040A and 1040 - which one you use depends on how complex your return is. You might also be required to file additional forms or schedules with the return, again depending on the complexity of your return.

A refund is just sending you back an overpayment. So if you had too much withheld from your paycheck, you'll get a refund of the extra.

There is also something called the Earned Income Credit, which under some circumstances can give a refund even if you didn't overpay. It's given to low income working people, or sometimes to low to medium income people with dependent children.

One must file his/her tax return on form 1040EZ or 1040A or 1040. Then if your return shows a tax refund, you will automatically receive the refund amount within a reasonable period unless you opt for adjusting the refund amount against next year's estimated taxes.
After filing your annual return, you can check the status of your refund amount on www.irs.gov or call IRS.

Depends on how you're going to file. You need no matter what a 1040. Children: EIC forms, self employed: Sch C, medical deductions, charity and mortage: Sch A and so on and so on... It depends on your tax situation which we don't know from your question. It would help if you would explain further in your question. Like how many children do you have? Do you collect SSI? Are you on unemployment? Do you collect out of a retirement fund ? Are you self employed?
