I have a similar problem, if i get your question right. When you have babies, it rips the hell outta your abs and they dont fuse back together. Its herediatary, my sis had hers fixed, as did my mom. I havent had mine done yet, because it cost my sister 10k over 8 years ago, so Im afraid to check into pricing. What you need is termed 'abdominoplasty', where they restructure the abs and cut out excess skin and reformat the entire abdomen and belly button. Both my mom and my sister said it was worse pain than childbirth. I had a lot of luck with mine using a cardio-fit machine (about 100 bucks, or cheaper at a thrift shop...you know those machines dont last long in everyones home). I too have trouble with my back, upper near my bra line. When you have a comprimised muscle structure in your figure 8 (runs from your bra line on your back to your abdomen), you will have muscle issues...lot of pain. Also, i found acupunture to be awesome. I was a total critic, but my doc suggested it and it works wonders. Havent had to go back for years, and I previously couldnt hold my arms out in front of me for more than a minute without experiencing excruciating pain. Email me or IM me if you want. Ive dealt with that for over 8 years now....and dont have pain (anymore).
also...I do this, and it really works, I do it all day long...
1. Abdominal Exercises
Traditional abdominal exercises can cause back problems. Sit-ups, leg-raises and other exercises fail to train the abdominal muscles in the most effective way.
The safest and most productive way to train the abs is to use the brace and hollow technique. Try this easy to use routine first:
Standing normally, breathing normally, suck your stomach in towards your spine and hold, remember to breath normally, hold for seven seconds and release, now do this exercise 10 times.
This is a basic brace and hollow routine that helps strengthen the abs and the muscular girdle that surrounds your waist. More advanced techniques can be found in my eBook.

hope this helps

Names of insurances? I'd suggest you to look for names of doctors or plastic surgeons who can help you. Are you shopping for health insurance? Most policies don't cover cosmetic surgery, and if you're just buying a policy, you'll have a waiting period. Have you tried wearing a support garment, such as a girdle? That might give your back some relief.
