It doesn't tell us any of the above. There are lots of reasons why people do not go see doctors. They could be leary of them and prefer to treat their symptoms themselves by using holistic methods. They could be part of a religion that does not believe in doctors. They could be too poor and have no insurance to help their medical bills. They could simply not care how they are feeling and don't want to be bothered with a doctor or could have even had a very bad experience with a doctor and just want to take their chances if they are sick. They could even have all of the above and just not know it because the symptoms aren't showing. They question is too broad for a single answer. My brother has never been to doctor since he was a small child getting his childhood shots. There is no specific reason other than he just hasn't gone to one. He isn't particularly in excellent health but that doesn't mean he has anything wrong with him either. He just doesn't see the need to go.

I don't know what's prompting this question, if it's home work I'd have a serious discussion with your instructor. Any assumption about the absence of a visit would be share speculation and conjecture.
