It's a small doll which is used in voodoo rituals. It is also a modern interpretation available in new age bookstores, and used in various witch spells. Generally it is assumed to be to harm the person represented, but in fact in voodoo tradition it is just as often to help them. I do not know how the traditional voodoo people make their dolls, but I suspect there is considerably more to it than the small stuffed gingerbread style characters sold in new age stores.

It is a doll that either somewhat mimics a person or some type of witch craft is used and the doll acts as someones body. So if you stab the doll in the leg with a pin that person will feel the pain.
Its not real of course and is rather lame even in movies and shows.
