you dish up what you eat. As a parent it is your job to supply the food, his to eat it. You do not make seperate meals for him. He will not starve himself. Mac and cheese ,grilled cheese and hotdogs do not make a nutrishous meal. I have a fussy son and dish up tea and it is his choice to eat it. If not it is straight to bed and nothing till morning. You know what it is getting better. We still have the occasional battle (you can read about one in my questions, it was one of those where I flipped out and took it on as a battle...big mistake but sometimes us mums snap) but after 7 years (he got fussy at 17months) it is finally getting better. There is light at the end of the tunnel if you do not offer alternitives. He eats what you eat or nothing. He will get better. Meat and 4 veggies is good. It is healthy and has the vitamins and iron he needs. The occasional night of things like mac and cheese are occasional but no more than once a week

About 16 ounces of whole (full fat) milk a day is plenty. Other than that, just offer healthy foods throughout the day at regular intervals. Have breakfast in the morning, followed by a mid morning snack, then lunch, followed by an afternoon snack and then dinner and snack before bed. Allow him to eat as much or as little as he wants at each meal or snack and then he will have to wait until the next meal or snack. Offer plenty of fruits and vegetables, whole grains, meats and nut butter. Just let him decide how much to eat, and he will eat what his body needs. My son really likes a smoothie (whole milk, yogurt, banana and strawberry) before bed and it helps get some extra calories in him (he is small too).

Do they like mashed potatoes? Try hiding a variety of things in them--like vegetables, corn, broccolli. My son went on a bread kick for months and would barely eat anything else. I just kept making a billion types of muffins and biscuits! He also went through a pasta phase--I would add different things like cheese to make it attractive to him. My son likeds chicken fingers, tator-tots and french fries (you can make these yourself from potatoes to make them healthier), peanut butter sandwhiches.
You could talk to the public health nurse in your area. They would be able to give you pamphlets with tons of different ideas!

how does a toddler get hold of hot dogs, do you realize what they are made of?
all the organ meats and leftovers are put into hotdogs: brains, liver, lungs, kidneys, testicles...Feed your kid the same thing you eat, place it in front of him/her, cut in small pieces, and do not play games. There is no such thing as "picky" eating unless you create that situation. If the kid does not eat it, throw it away, do not try and push. Do not mix food and emotions.
