Quien= Who
es = is
y = and
donde = where
nacio = born

So if you were to translate this, you would say in English:
Who is s/he/you and where was/were s/he/you born?

The reason why you don't see the pronouns before or after the verbs is because in Spanish verbs are conjugated differently for every pronoun (I, You, she, he, we, they, etc.). So the verbs to be in this case are:

Pronouns: Verbo: Ser Verbo: Nacer
Present tense Past tense (preterito)
Yo soy naci
Tu eres naciste
El/Ella/Ud. es nacio
Nosotros somos nacimos
Ellos son nacieron

Thus, I would answer the question this way:
I am a teacher and I was born in San Francisco.

Soy profesora y naci en San Francisco. OR
Es profesor/a y nacio en San Francisco.

Hope this helps.

Yeah... Your ex-buddy sound like an entire ignorant. it is idiotic to attempt to take benefit once you realize somebody else does not talk the language... Hmm... besides... I could be "grasa"... yet double "s", nicely... the observe does not exist. although; "grasa" potential like fat; yet no the adjective... the noun. like the element that the healthcare expert takes away in a tummy-tuck. Hmm... i've got by no potential heard that as an insult.

Politely it means:

¿ Who are you and where were you born?

but as we don't have the subject , it also could be:

¿Who is he/she and where was he/she born?

but i would say the first option is the most usual...

It means: Who is (s)he and where was (s)he born?
or, using polite language:
Who are you and where were you born?

Shouldn't the word "nacio" actually be "nacido"?
