I don't know how big or small you wanted the tattoo to be, but what about using it as a background for sleeves? That way you could have one arm decorated with say, a traditional Japanese style of smoke, and the other with flames... or you could tattoo just one arm with flames that turns into smoke and rides up your arm/neck or twists onto your back.

I have a korean dragon with cherry blossoms, water ball, ghost flames and smoke on my chest. I like the idea of the smoke being a negative space element. If you want something with fire and smoke that covers a lot of different terittory. Do you want something that is Asian inspired or traditional American? Just go with whatever you like and try to merge fire and smoke into it. You can pick anything you want to set on fire and fire leads to smoke. Just be creative and inspire yourself. Before you get the tattoo put a picture of it up on the fridge for 6 months and if you can live with it there you can live with it on your body.
