I usually pick a frame that fits the painting not the other way around.

I'll say the best way to do it is using a stencil, which you can create your self. You can get stencil paper at a craft store and cut the shape or just do it directly on the wall with painters tape. Draw the lines on the wall first with a pencil using a level so it's straight. Buy stencil brushes or with a sponge, apply the paint by tapping not brushing so the paint doesn't run behind the paper or tape. If you use tape, be sure to take it off as soon as you finish painting to avoid pulling dry paint. If the paint runs a little, have a wet cloth handy to wipe it off as soon as possible. When you finish making the frame shape, do the shadow. When completely dry (Better after 24 hrs.) Mask the painted shape with the tape, leave a space along side the frame line, and forming the shade shape, put tape at the other side ( forming the shade). That would be at the bottom side and the right side of the frame stripe. At the corners the shade will conect to the frame on a 45 angle. You can use acrylic paints and thin them with glaze. This will give you more time, before it dries, to get the finish you desire. To paint this frames would take very little paint.

Well it's for him... and I'm going to assume that it's a picture of the two of you together.
Well, it depends on where you are. If you're at the zoo, put some animals on it. If you're in a park put on some flowers, that kind of thing. And don't be afraid to use something other than paint too. Hobby Lobby is great for crafty kinds of things. But paint's cool too. :)
If the background doesn't have a color paint it his favorite! Or paint it stripes of his favorite color.
Keep in mind that things don't have to be symetrical, and sometimes randomosity (if that's even a word) can make things more fun.
As for designs squiggles are always fun! Stars are great too, as are hearts as suggested by another user. Another thought is putting on some of his favorite things, or things that remind you two of the time you've been together. Also, words are always a thought, maybe words that are important to you, or inside jokes. The target theory is also interesting. (a circle of one color, surrounded by a circle of another color and maybe another. As many as you want, making it look kinda like a target) Sunshine is also a thought, as is a checkerboard pattern.
Other thougts for shapes are animals, simple geometric shapes (squares, circles, diamonds, octagons even!) happy faces, stars, and things like that.

the best picture frames i've ever done were done with a material called Gold leaf, its very easy to use and it makes your frames look completely gold. without pain marks or streaks
