I've been out 11 months and now on long term disability

If the has enough employees to have to abide by the Family Medical Leave act then no they can not fire you. if it is a smaller company than yes they can terminate you.

If you didn't fill out your FMLA paperwork, yes.

Not if you're on FMLA. But I don't know how long it is good for.
