Get the kitten to a vet. He sounds sick.

Take him to the vet. He may simply need a good deworming. Also, if your sister has been changing his food from what the breeder/shelter offered or is offering lots of treats, don't. Just feed what he was eating before and let his tummy settle. Blood can be from a parasite, from an irritated bowel or from more serious disease. It can be common, so don't worry, but do go to the vet now.

Kittens and puppies are often born with parasites in their digestion tract, take them to the vet for a general dewormer. It may take a few doses but it should clear up. If the kittens are eating well and playful then I am 99% sure its just a parasite.

This is not good news. Get the kitten to a vet immediately. Sounds like distemper, feline leukemia or HIV.
