No! they never know! My first husband was the same; he worked on a building site; the only thing he washed on a daily basis was his face and hands; he showered once a week. I fought an uphill battle to get him to change his underwear every day; he only got the hang of daily showering the year we broke up. He was raised in a house with no bath or shower, and never learned good hygiene.
As for your flatmate, you will just have to be direct, and to the point - don't drop hints, it's a waste of time. Just say out straight "listen, if we are going to live in harmony, we are gonna need to have a talk. About keeping clean, and not smelling up the place. The bottom line is, you just have to start showering every day and wearing clean clothes every day, or no-one is going to want to live with you."

Oh lol that's disgusting! Sorry that's just bad! Yeah I'm pretty sure that with today's society that he's aware of his lazy showering habits causing issues for others.
What you can do is put deoderants and soaps on his bed from time to time. When he asks if you did it just say no but wow you do love that scent and you bet it would smell lovely on him. He should give it a shot! See how that works. The best approach to something like this is positive instead of negative. Suggest scents that you think would work well with his chemestry etc instead of saying WOW YOU STINK. That eliminates defensive behavior. He'll get the clue eventually or you might have to invest in some nose plugs bless your heart!

They truly don't know they smell badly. Their noses have "turned off" for lack of a better way to explain. It's like a farmer who no longer smells the animal waste because he has to smell it every day.
His ignorance about proper hygiene is a totally different matter. Was he brought up in a cave? Geesh!
That type of person needs to be told directly, in no uncertain terms, that he stinks and needs to clean himself up because he is offending everyone else around him. He also needs to be told that his lack of hygiene is making him more vulnerable to illness and infections.

Be blunt: Tell him, "either you go or that smell goes! You stink like a hog! Guy get your *** in there and get a shower and wash those stinking filthy clothes. And start taking a shower at least once a day. While you are at it brush those nasty teeth.

The answer is no, they don't know they smell bad.
I had a guy friend who also smelt really, really bad, and even worst when he puts aftershave. I thought he would know that he smells bad but he doesn't know. I had to use my tissue and own perfume on my hand to avoid the smell.. but only by abit.
So me and my friends told him and then he never talked to us for a while. But a few months later, he started talking to us... only abit though.
But I think it's wise for you to tell him as it would just make you suffer even more.

It may "not be right" to have to tell him, but you do have to -it is very inconsiderate of him to live like a swine if he shares your flat.
You can try to leave hints like a bar of soap on his dinner plate, or say loudly when you have friends over " Whew, who is frying baloney?" and wave your hands around him...or try the direct approach and say to him " I know you don't like to bathe, but your stench could knock a buzzard off a schitt wagon"

just tell him that he stinks, and that he's a sweaty git. He should be showering every day. (Twice daily is even better)
Tell him that neither of you stand a chance of getting any women back to the flat until he cleans himself up

just be polite and really composed and all of a sudden just shout in his ear get yourself a bath you dirty bastard you smell like a skunk//he'll slink off to the bathroom and wash under his arms and tell you he's had a good bath and while he was in there you chucked all his clothes in the bin so your half way there

Try to get a place of your own or share a flat with someone who is cleaner. I do not think you can put a word across to him about this without hurting his feelings.
