I know Puerto Rico and The US Virgin Islands. Anywhere else?

Canada and Mexico (if traveling by car), U.S. territories, and some Caribbean nations. Here's the list, where you can check each individual country:

I believe the deal is that you can get INTO Canada without one, but to get back in the US you need a passport or proof that you have applied for one.

That'll probably tell you more than you ever needed to know :)

There's evidently an incredible backlog of applications right now, but you should go ahead and send in your application anyway so you have it when you need it in the future.

Canada and Mexico, but I'm not sure for how long they will continue that. You do need a Birth Cert though.

i know for mexico u dont need the actual passport, but you do need some paper of identification and from the INS

Canada for now... they are soon going to require one for there too... unless of course you go the back way... there are many of those... there is one right by my friends house actually...

u need a passport to every country u go because its your identification.
