The first one is a Christian who is also an American, whereas the second one is an American who is also Christian.

I would say the first one suggests that the person who identifies themselves as this considers religion to be more important than nationality, whereas the second one has that reversed - nationality is more important.

The first one also has connections in my mind with a fanatical Christian, whereas the second one presents the idea of the person being calmer about it, more just happening to be Christian rather than have it seriously affect their lives.
