To begin with, I want to mention something here: the term "Witches" was not adopted by people following the Pagan path. It was a term denoted by Christians, during the burning times, and it has simply followed Pagans ever since. Most Pagans would not term themselves as a Witch. We do not fly on brooms, we do not have the ability to make someone disappear in a sparkly plume of confetti. Now with that said, I will answer.
When doing any spellwork (same as praying) I typically do it in front of my altar (the Family Bible talks of putting up an altar of God as well), which currently is set up in my living room facing North. Other times in my life, in other homes, when I've had an attic or a basement, my altar would be set up there. More privacy, less distraction. It has always faced North. North signifies growth. I will also add, that 'spellwork' is a lot like 'prayer'. We are asking the Divine for growth, healing, assurance, answers, etc. I have never cast any spells to hurt someone. However I have cast a spell to guard me from being hurt by a particular individual. And doing such a spell is no different than "The Armor of God" [Eph. 6:11].

The timing has to do with the moon. When it's waxing or full, it's for spells that bring something to you. Waning or New, it's for banishing spells.
I suppose the best place is somewhere outside, but anywhere is fine. I cast in my bed room.

Time depends on the spell. Usually I do them when I have good weather, assuming I can get outdoors. (In the apartment, obviously I have to work indoors. We don't exactly have a lawn.)
After that, I prefer full moons, so I can work at night and have plenty of light. But if it's something quick and simple like visualization, I can do it anywhere, anytime.

I have an altar in my living room--I cast whenever I feel the need, because your need and intense emotion are what drives the spell, and waiting for the perfect planetary hour can seriously diminish that

Wherever I feel like but usually in my home after I've created sacred space. Best time varies on the circumstance. Best place doesn't have much to do with it.

I feel more confortable with doing it at home sice I live in a big city and there are people everywhere who like to call the police on EVERYTHING!!! So, I do it at home, usually at night because it just feels more powerful (to me) under candlelight, but that is just me.

I cast mine at home when I'm alone, because it's intensely personal and private. Sometimes I do it while I'm cooking, though, but silently.

Wherever I happen to be when the need arises.
