Most DMV's require a birth certificate and Social Security card. You can get a copy of your birth certificate from the Health Department where you were born. Make sure it is a certified copy. It may take up to a month to get it. Some areas have an online service. Depending on your age, your parents need to sign also.

To make sure you can be trusted to be on the roads alone.

You contact the Health Department in which the county you were born.
